Frequently Asked Questions

What is therapy? Therapy, counseling, psychotherapy, three words, that have the same meaning. This is the process of meeting with a mental health professional to work forward movement and growth by examining and gaining insight into life choices and difficulties faced by individuals, couples, or families. Therapy can be used to work on feelings, beliefs, relationship concerns, problematic behaviors or healing from trauma or troubling past experiences. Therapy will help you to gain a deeper understanding of your thoughts, emotions, and behaviors through a collaborative relationship. In this relationship you will be provided with support to achieve goals and work through what stands between you and your full potential.

How do I know if therapy is right for me? Therapy is a wonderful and powerful tool that can be used by anyone who may be looking to improve their life or relationships, as well as boost confidence and communication skills. Therapy can benefit those looking to make positive changes, reach a new level of success or higher level of functioning.

How can therapy be helpful? There are countless benefits to therapy. The efficacy of therapy and how it will be impactful for you varies based on a variety of factors. Some of the known and proven benefits of therapy include but are not limited to: ï‚· a new perspectives ï‚· feeling acknowledged and supported ï‚· improved self-esteem and confidence ï‚· improved communication and problem-solving skills ï‚· a better understanding of yourself, daily struggles, strengths and areas of improvement ï‚· development of new coping skills ï‚· development and enforcement of healthy boundaries ï‚· developing new conflict resolution skills ï‚· better sleep ï‚· feelings of empowerment

How long will I be in therapy for? Therapy is not one size fits all so your time in therapy will depend on your needs and goals. This can be something that is determined with your therapist and can shift thorough the process.

How long is an appointment? Our therapists reserve 50 minutes for each session.

What can I expect from my first session? The first session is different from subsequent sessions as it is an initial evaluation. During this session your therapist will provide a comprehensive assessment which will take into account numerous factors such as your personal history, current concerns or struggles as well as what you are hoping to achieve through the therapeutic process. If you already have goals in mind you and your therapist can discuss this as well. This is when you and your therapist will work together to develop a plan for your time together. Be sure to ask any questions that you may have at this time. Therapy then begins in the following session.

How often should I see my therapist? We recommend that clients begin with seeing their therapists once per week. This is supported by not only our professional experience but clinical research as well and is most associated with a positive outcome for building rapport. During therapy this frequency may be increased or decreased based upon your individual needs.

Administrative and Logistic

What are your appointment hours? Hours vary by therapist, but we offer sessions during the day, as well as in the evening (after work) as well as on weekends.

Where is your office located: We have 2 locations. The Chicago location is in the Lakeview neighborhood at 2845 N. Sheridan Rd. The office is in the Presence Center for Advanced Care which is connected to St. Joseph Hospital on the Southeast corner of Sheridan and Surf. We are in suite 708. The Fort Wayne location is at Hobson Buisness Suites at 3601 Hobson Road. The building is at the Northwest corner of Hobson and Collosium and we are on the second floor in suite 205.

Is your Chicago office accessible via the CTA? Yes! The closest train station is Diversey (Brown/ Purple line) and is approximately 0.8 miles away. You can also access us from the Belmont station (Red line) which is one mile away. We are also accessible via bus by using the #77 which drops off in front of St. Joseph’s hospital, or the #126 and #151 which both stop in front of the building.

Is there parking available? Yes. At the Chicago location, there is garage parking on the east side of the building (Parking 4) on Commonwealth St. It can be validated on the first and sixth floors for $5.  There is also valet parking if needed. There may be street parking available on Lakeview, Diversey, or any of the side streets but be sure to check signs carefully before parking. Some clients have used SpotHero to find great parking options as well. Give yourself a little extra time to look for a spot, if this is your first visit. In Fort Wayne, there is a paking lot in front of the building.

Do you offer tele-therapy? Yes we do! All of our therapists use a HIPPA compliant video service for Illinois & Indiana residents who desire tele-therapy.

Do you offer in person sessions? Yes, some of our therapists do offer in person sessions at this time. Please notate your preference when you speak with our intake coordinator so that they can match you with a therapist who is seeing clients in office.

Are any COVID-19 precautions being taken at this time? Yes, as our Chicago office is located in a medical building masks are required within in the building. Once within our suite you can remove them if you are comfortable doing so. All therapists who are providing in person sessions have been vaccinated against Covid-19. If it would make you more comfortable your therapist can wear a mask in session, just ask!

Do you take insurance? Yes! We are currently in network with Blue Cross Blue Shield PPO, Anthem, Blue Choice, Parkview Signature Care, and Blue Cross Blue Shield HMO - AMITA group. We cannot guarantee coverage but are happy to do a complementary check. Prior to your initial session, we recommend that you call your insurance provider regarding your coverage for mental health services and ask for a benefits verification. Inquire whether or not you are in network with us. We also recommend asking whether or not you have a deductible and what your benefits cover. You can find the number for member services on the back of your card, this should take fifteen minutes or less of your time. In some cases even if you have Blue Cross Blue Shield PPO you may not be in network with us. At times your plan may use a third party (Optum/ United Behavioral Health, Magellan, Compsych) to cover behavioral health benefits. Unfortunately, these are considered out of network.

What are some common insurance terms that I may hear from your office? Copay- A fixed dollar amount that is your responsibility to pay to the provider per session at the time of service. Copayment rates vary from plan to plan. Insurance will pay the provider for the remainder of the session fee. Co-insurance- Co-insurance is usually a percentage of the total bill (for example 20 %) . This is your responsibility to pay directly to the provider, insurance will provide the rest of the payment. Deductible- The amount that you will need to pay out of pocket before your insurance company begins to pay for care. This amount will vary by plan and will reset on an annual basis. Should finances be a concern please discuss this with your therapist.

Can I still work with you if I do not have insurance or am not in network? Yes! We can provide you with a self-pay rate.

Can you provide me with a quote for cost of services? Our standard fees are: $250 for the initial session and $200 for subsequent sessions. However if you plan to use insurance, your cost may vary and we can provide a quote. Please keep in mind, the quote we give is not a guarantee of what your insurance will pay, the best source for that informtion is your insurance company.

What forms of payment do you accept? Evolve Wellness accepts payment by all major credit card, check and cash. Any amount not covered by insurance is your responsibility. We do require that all patients keep a credit card on file.

What is your cancellation policy? Evolve Wellness requires 24 hour notice to cancel a session without charge. You will be charged the full fee ($150) for any missed sessions or sessions cancelled within the 24 hour window. This fee is not covered by insurance and is your responsibility.

Can your prescribe me medication? No, we do not have a prescriber on staff at this time. We can assist you in finding a prescriber should this be necessary.

Do you offer neuropsychological assessments? Unfortunately, we are unable to offer neuropsychological assessments at this time, we suggest you reach out to your insurance provider for a list of in network providers that do neuropsych testing.

What languages are services offered in? We currently offer services in English and Spanish.

Will you be in communication with my other providers? Therapy services are covered under HIPAA, therefore what you share as well as your records are confidential. Should you request that we communicate with another provider we will do so upon completion of a release of information.

How do I schedule an appointment? If you are a perspective client you can reach our intake coordinator by calling (773) 906- 3225 (if in Chicago) or (260)740-3324 (if in Fort Wayne), filling out the contact form, or emailing admin@evolve-well.org . If you are a current client please reach out to your therapist and they will respond when they are able.

How do I choose the right therapist for me? Each of our therapists is unique in their own way and agrees that a good fit is important to a successful therapeutic relationship. Please take time to read our therapist bios and speak to our intake coordinator who can help match you with the right therapist.
